Your initial appointment will consist of an examination and review of your treatment options.
Dr. Karlin will conduct a thorough review of your medical history and an oral cancer screening. Prior to treatment Dr. Karlin will consult with you about your diagnosis and treatment plan. Additionally the front desk staff will discuss treatment costs and insurance coverage prior to beginning treatment. We want to make sure you have every opportunity to ask questions and understand your treatment needs.
Please assist us by providing the following information at the time of your consultation:
Any x-rays taken by a previous dentist, if applicable.
A list of medications you are presently taking.
If you have dental insurance, please bring your insurance card and referral, if needed.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. artificial heart valves and joints) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our facility.
If you should need to cancel and/or reschedule your appointment we request a minimum of 48 hours notice so your appointment spot can be offered to another patient. Cancellations with less than 48 hours notice may be assessed a cancellation fee.
